This local authority based in the South Wales is seeking an experienced Social Worker to be part of Adults Community Team. The Adult Social Care service has strong social work values and a focus on improving outcomes to vulnerable adults and their carers.

We are seeking an experienced Social Worker to support older adults; adults with physical disabilities; adults with Dementia and the families / carers of these individuals.

You will manage a caseload and practice in a person centred and outcome focused way. Undertaking assessments, producing, and reviewing care and support plans will be fundamental elements of your role.
