This position will appeal to OTs seeking a diverse community-based practice serving clients across the rehabilitation continuum. Duties include:
Hospital discharge planning and assessmentsCognitive assessments & rehab to increase client independenceGraduated activity plans to increase home/community functioningDevelopment & implementation of community integration plansCase management to facilitate client access to other health servicesDevelopment and supervision of rehab assistant plansLiaising with funders, family and healthcare team Qualifications
