CARE NAVIGATOR at Corona Regional Medical Center 

The Care Navigator reports to the Director of Emergency Services and serves as an integral team member of a statewide initiative to improve access to Medication for Opiod Use Disorder (MOUD). This role conducts initial brief assessments, introduce patients to MOUD programs and services, expedites appointments at Medication for Addiction Treatment (MAT) capable clinics, serves as the primary coach for their clients , and maintains ongoing contact with the California Bridge Program panel. The Care Navigator assists with access to other services such as financial counseling, primary care, mental health services, social services and residential treatment facilities to link patients from the hospital to the community.
