Job Description

• Providecare coordination services to patientsenrolled in chronic care management (CCM),principal care management (PCM), and remotepatient monitoring (RPM)programs.
• Conduct telephonic assessments and develop care plansbased on the patients' needs and goals.
• Monitor the patients' health status, adherence tocare plans, and progress towards desired outcomes.
• Communicate with the patients, their caregivers, andtheir primary care providers regularly andeffectively.
