The role involves assessment, treatment planning, coordination of care, and advocacy to facilitate successful community reintegration while reducing recidivism. The position requires collaboration with correctional facilities, community agencies, and healthcare providers. The Licensed Clinical Case Manager (LCCM) will provide project oversight, clinical assessments, and brief interventions to the participants in the program.
