About the Role

• Supporting intake and admissions activities across Support at Home and CHSP programs
• Responding to client enquiries and referrals in a professional and timely manner
• Conducting outreach visits and meetings with prospective clients and families
• Assisting clients to navigate My Aged Care and access appropriate aged care services
