Clinical Care Partner - Supporft at Home

We are currently recruiting on behalf of a highly regarded not-for-profit organisation committed to providing responsive, compassionate and person-centred care to older Australians.

We are seeking an experienced Registered Nurse with a strong Care Advisory background within Home Care Packages (HCP) and experience supporting the transition into Support at Home (SAH) to join a passionate and supportive team in a full-time permanent opportunity based in Melbourne's north-east.
