Clinical Social Worker

Responsibilities

• Take care‑management responsibility for vulnerable clients, including production of a comprehensive social assessment of client needs, formulation of a social care plan and identification of relevant resources to meet the individual’s needs.
• When appropriate, following assessment, co‑ordinate the delivery of social care plans and liaise with the appropriate government or charity agencies.
• Work alongside and in partnership with individuals, families and caregivers using a range of interpersonal and communication skills to assess highly complex psycho‑social needs, set objectives, develop care plans and deliver appropriate interventions.
• Assist the multidisciplinary team to recognise and intervene as necessary for high‑risk patients using the approved Social Work Criteria for the Assessment of Patients with Social Issues.
