Community Mental Health Nurse

One of our leading NHS clients is looking for experienced mental health nurses to cover the shifts in the community hospitals in Wales. We are seeking applications from Registered Mental Health Nurses who have experience of working in community services.

Description:

The post holder will be responsible for the assessment of care needs, the development of programmes of care and the implementation and evaluation of these programmes of care for a defined clinical caseload including the role of a Care Coordinator within the Care Treatment Planning under the Mental Health (Wales) Measure .

The post holder is required to work without direct supervision for their delegated caseload. The post holder will have effective communication skills and develop positive working relationships between all healthcare professionals, statutory and voluntary agencies.
