To support the care management teamConduct client and care professional introductionsOne day per week dedicated to supporting the scheduling teamParticipate in on-call duties as required. Compliance with Home Instead's Equality, Diversity and Equal Opportunities policyBuild and maintain positive relationships with potential and existing clients, their families and other professionals involved in their careAudit client care plans and assist with digital care planningMaintain accurate client and care professional records on the Home Instead softwareCarry out client reviews and action these accordinglyAbility to cover frontline care visits as part of the on-call roleCarry out any other duties deemed necessary for the successful operation of the business Qualifications
