**Job Summary:**

CarePartners is seeking an Advanced Practice Provider to join our **Program of All-inclusive Care for the Elderly (PACE).**

**Qualified Candidates** :

+ Completion of an accredited nurse practitioner or physician assistant program with appropriate certification exams
+ This is an independent role and candidates must have experiencetreating geriatric, internal medicine or mental health patients
+ Please note that this position does not offer visa support

**Position Highlights:**

+ Schedules is Monday – Friday 8 am - 5 pm with on-call coverage
+ Performs procedures necessary to implement plan of care; consults with interdisciplinary team regarding patient assessment findings
+ Discusses patient plan with patient and family
+ Develops and revises patient education programs and evaluates effectiveness of learning
