Health Care Social Worker - Hospice Unit

The Social Worker reports to the Health Services Manager and works with the Interdisciplinary team, and management to plan, deliver, and evaluate a comprehensive social work service to the Hospice Unit.The social worker is governed by the Social Work Act, The Social Worker’s Code of Ethics, and relevant legislation and works collaboratively with other agencies.
