We are seeking a compassionate and proactive Intake Assessment Coordinator to be the welcoming first point of contact for clients and families entering our Support at Home programs.

What you’ll be doing

• Leading intake & assessments — processing My Aged Care referrals, conducting face‑to‑face assessments, and ensuring every client receives a respectful, person‑centred entry experience.
• Building strong partnerships with hospitals, pharmacies, service providers, and community organisations to strengthen access and collaboration.
• Supporting client independence by identifying needs, setting goals, and ensuring timely commencement of services.
