License required
ASW, LCSW, or AMFT/MFT license as a social worker.
Conducted comprehensive psychosocial and risk assessments for acute behavioral health patients
Developed and implemented individualized safety plans and crisis interventions
Facilitated family meetings to support treatment goals and discharge readiness
Led therapeutic group sessions within an acute inpatient setting
Coordinated discharge planning, including post-acute referrals and continuity of careS
