Overview
Catholic Health Home Care is looking for passionate, dedicated LPNs to provide comprehensive care to patients at home.
Team 7: Belle Terre, Centereach, Farmingville, Head of the Harbor, Lake Grove, Nesconset, Nissequogue, Old Field, Port Jefferson, Port Jeff Station, Setauket, St. James, Stonybrook, & Terryville.
+ Weekend Day shift: 7:00AM to 5:30PM
+ Provide 16-18 visits per weekend- ensuring coverage of high priority cases
Job Details
Our Home Care LPN’s:
+ Case manage, assess, plan, develop and implement a comprehensive home health care plan for their patients.
+ Measure vital signs, Administer IVs, change dressings, clean wounds as needed
+ Provides skilled nursing care, preventative rehabilitative procedures, and prescribed treatments with a variety of patient populations within various potentially complex home situations.
