Medication Historian

A medication historian conducts patient interviews within the hospital and emergency department to obtain medication lists. A standardized questionnaire and process to obtain a complete medication history is used for each patient. This history is a comprehensive list of prescription medications, infusions, over-the-counter products, immunizations, dietary and herbal supplements which are entered into the electronic medical record. The medication historian technician will use various resources to gather this information, including calling physician offices, dialysis centers, pharmacies, hospitals and nursing homes and obtaining medication administration records to confirm outpatient regimens
