Mental Health Clinician - Social Worker II - Center for Gender Affirming Care

This position provides professional services to patients and families at the Center for Gender Affirming Care, which facilitates the use of individual, public and private resources so that optimal functioning can occur for both the patient and the family. These core services include assessment support needs and resource provision, mental health treatment and community resource planning. Mental Health Clinicians collaborate with many disciplines, programs, departments and organizations in the accomplishment of these duties.

Experience working with Transgender, Non-Binary and Gender Diverse children, youth and families required.

JOB SUMMARY:
Social Worker II (LCSW/LMFT/LPCC) applies principles of Social Work and other behavioral health practices to provide patients and families with assessment, interventions, support, advocacy, and case management as a primary provider and/or as ancillary support to the primary medical provider.
