Mental Health Social Worker – Adults

To discharge local authority duties in respect of adults in the two psychiatric wards at Wedgwood House in Bury St Edmunds.

This would include assessments under the Care Act and Mental Capacity Act. As well as supporting with safeguarding enquiries, supporting discharge planning and sourcing placement or care packages, liaising with the community mental health teams to support follow up, and applying for funding or grants as needed to support discharge.
