Help at Home by Montefiore is a unique service that is one of Sydney’s greatest offerings to clients that require support so they may maintain independence in their own home for as long as possible. Our team of carers provide around the clock care to each individual client and work closely with our dedicated team to ensure the right care is supporting the client. 

Due to internal movement, we are looking for an experienced NDIS Support Coordinator to join our collaborative environment, this is a remote role where you will be on the road working with our NDIS participants across Greater Sydney.

This is a fantastic opportunity to work with participants to maximise the value of their NDIS plans, build capacity, and connect them with services that support their goals and independence.
