Nurse Care Manager (RN) - Obstetrics

Position Summary:Joining our team at Plaza del Sol Family Health Center in Corona, Queens, the Nurse Care Manager (Registered Nurse) works to support primary care patients that were referred for Obstetrics (OB) Services and is a liaison to facilitate patient/provider communication. The Registered Nurse performs required assessments and provides direct patient care services to targeted patients to ensure they adhere to the treatment plan, including patient education. The Nurse Care Manager develops, evaluates, and implements individualized Care Plans in accordance with the health care needs as directed by the PCP. Educates the patients and their families on how to follow their care plans and updates them as needed. Provides linkages for services including but not limited to Nutrition Services, Mental Health Services, or Case Management.
