The Primary Care Provider (NP / PA) will:
Provide compassionate, holistic patient care, addressing diverse needs and collaborating with multidisciplinary teams Deliver exceptional clinical care in-person and virtually, with in-person settings including private homes, congregate living facilities, and/or other home settings Integrate practices and processes to achieve company goals Maintain accurate medical records and participate in initiatives for patient engagement Collaborate with external providers and partners Perform other duties, as needed Qualifications: Graduation from an accredited Health Professionals school NYS Nurse Practitioner (NP) and/or Physician Assistant (PA) license Successful completion of national Board Certification Active DEA licensure and state-specific Controlled Substance registration, as appli...
