Nutritionist

Responsibilities

• Assess, recommends and implements per established and approved disease-specific or condition-specific protocol orders from the referring practitioners, executing interventions to meet individual nutrient and energy needs, including but not limited to prescribed diets, modification of food textures for dentition and individual preferences; nutritional supplements; nutrition support therapies, for example enteral nutrition (tube feeding) and parenteral nutrition support.
• Works closely with interdisciplinary team members for care coordination (e.g. ability to swallow, in the case of patients with dysphagia with rehab, occupational therapist, speech language pathologist, consulting on drug-food interaction with a pharmacist) and provide referral to other health professionals according to the needs of patients.
