The Occupational Therapist - Home Health provides in-home patient care across the west side of the Cleveland area (including Lakewood and Tremont), supporting a diverse population with orthopedic, neurological, and cardiovascular conditions. This role focuses on evaluations, start-of-care visits, and ongoing treatment, including discharge planning, wheelchair assessments, and durable medical equipment recommendations. Caregivers educate patients on safely completing daily activities to remain independent at home while managing their own caseload and following patients through the plan of care.
