Position Summary
The Registered Practical Nurse (RPN) Supported Attachment Nurse Navigation Program plays a key role in supporting the attachment of unattached patients to primary care providers across Northumberland. Working in collaboration with primary care practices, community partners, and the Ontario Health Team of Northumberland, the RPN facilitates a coordinated, client-centered onboarding process that improves access, continuity, and equity in care.
This role focuses on clinical intake ensuring patients are appropriately assessed, supported, and connected to the primary care provider.
