The PCT supports our nursing and medic visiting team during Hospital at Home visits through a paired visit model. The PCT is paired with a nurse or medic during their shift to complete Hospital at Home visits. These visits include management of the patient according to the medical and nursing care plan. Tasks that the patient care tech may complete include but are not limited to: Checking vital signs, reinforcing education on how to use the in-home technology for Hospital at Home, helping patients with ADLs, phlebotomy, and 12 lead EKG.
The PCT also supports the virtual command center when not out in the field doing home visits with such tasks as screening phone calls, stock/inventory of medical supplies, equipment, and food/nutritional items, and completing outreach to potential remote patient monitoring patients...
