Physical Therapist 

Salary Range: $(phone number removed)–$(phone number removed) + $20K Sign-On Bonus

This is a full-time, Monday–Friday position for a licensed Physical Therapist working with the geriatric population in a community-based Program of All-Inclusive Care for the Elderly (PACE), providing comprehensive rehabilitative services, chronic disease management, and mobility support to help frail seniors remain independent and safely age in place.

Job Overview:

As a Physical Therapist, you’ll play a key role on an interdisciplinary team that provides comprehensive, person-centered rehabilitation services for older adults.
