Daily schedule: Monday to Friday, 8:00 AM 5:00 PM

Practice setting: PACE Clinic

Types of procedures: Comprehensive geriatric assessments; management of chronic conditions such as diabetes, hypertension, COPD, and CHF; medication reconciliation; coordination of care with interdisciplinary team members; annual wellness visits; acute care follow-ups; advanced care planning

Number of patients: 2 5 per day

Ages: 55 and older

Additional information: Provider will work within a multidisciplinary team that includes pharmacists, occupational and physical therapists, nurses, and social workers.
