Qualified Social Worker (Children) - Grade J (Social and Healthcare Qualified)

Carry out social work tasks to a satisfactory professional standard in accordance with the policies of the Social Services Committee and the instructions of the Service Unit Head. Comply with and meet the standards set out by the policies and procedures jointly agreed between Social Services and Dorset Healthcare University NHS Foundation Trust.

Account to the Service Manager for the quality of work and provide information on the nature of the work.

To ensure that support plan reviews are carried out in a timely and person centred way evidencing need for continuing support.

 Undertake comprehensive assessments. Contribute to comprehensive risk assessment and risk management plans for adults. 

 Undertake care coordinator role for designated caseload in accordance with the standards set out by the policies and procedures by Dorset Healthcare University Foundation Trust.
