Responsibilities
- Make a first health and wellness analysis and an individualized plan of treatment
- Carry out medicines and assist with pain monitoring
- Clean and dress injuries
- Record signs, symptoms and vital signs
- Display patient health status and update care plan accordingly
- Advise patients and their households on appropriate home care
- Provide tips to boost safety at home
- Spot early signs that might lead to a hospital visit
- Supervise home health and wellness aides
- Coordinate with physicians, social workers or other health consultants
