Job Summary
Coordinate the care for oncology patients, their family and/or caregiver across the continuum of care while overcoming healthcare barriers. The objective is to coordinate and provide high quality patient care across all care delivery sites. This may include coordination, facilitating, monitoring, and evaluating interventions to achieve desired outcomes.Essential Functions

Evaluate all new patient cancer referrals to initiate contact with patient. Review patient records for NCCN guidelines for diagnosis, staging, and treatment as well as to identify staging studies, molecular profiling, and other tests that may be scheduled prior to the initial consult.
