Hospice RN Case Manager
Responsibilities:
Completes initial comprehensive and ongoing assessment of patient and family to determine Hospice needs Admissions of patient for services and development of the initial patient/family Plan of Care Uses health assessment data to determine nursing diagnosis Coordination of services with the patient, family, and hospice interdisciplinary team members Requirements:
Valid Registered Nurse (RN) license RN Diploma, Associates or B.S. degree in nursing Agency Hospice experience preferred Excellent observation, problem solving and communication skills Compassion and integrity
