Senior Change Facilitator

Join our Community Living Support team in Penrith. We work with people to build independence and confidence by focusing on their values and personal goals, while encouraging social connection and a sense of belonging in the community.

As a Senior Change Facilitator, you will be trained in the Motivational Interviewing change model and will work with clients and stakeholders to assist clients to make significant personal change. You will provide mentorship within the team while assessing individual needs, developing tailored change plans, and delivering flexible services. You will play a key role in building strong relationships, connecting people with community resources, and ensuring high-quality, compliant service delivery through accurate reporting and ongoing review of client progress.
