Opportunity Description
Summary:
With general supervision, provide advocacy, support, linkage, and coordination of services for an assigned caseload. Develop individual service plans in conjunction with assigned caseload. Participate in team meetings with participants and their service providers, and advocate for their rights and implementation of needed and desired services. The main goal is to assist the participants in maintaining and obtaining services that will assist them to live in the most integrated setting possible, within the community. Assist with facilitating workflow, clerical support for NHTD/TBI and OPWDD Waiver Programs. Mailing, copying, requesting needed medical documentation, discharge planning, assisting with treatment plan corrections, and implementing systems to improve efficiently within all Waiver programs.
