Job Responsibilities:
- Performs Patient assessments:
- Accurately assesses patient’s appropriateness for home care, learning needs and progress toward goals.
- Collaborates with patient, family, and physician to develop the plan of care for the patient.
- Performs baseline assessment of patient swallowing, communication, and cognitive abilities as indicated.
- Maintains safe environment for patients and co-workers:
- Assesses social environment and adjust or provide education as needed.
- Assesses for potential clinical risks and adapts program as indicated.
- Follows universal infection control measures.
- Implements treatment program to achieve designated goals:
- Administers occupational therapy services per physician orders.
