Social Worker, Inpatient

Summary: Under the Direction of the System Manager of Inpatient Behavioral Health Social Work, the Inpatient Social Worker will provide direct services to adult psychiatric patients on the acute behavioral health units at CCHP, including leading groups, individual therapy, family and systems meetings, completing intakes, case management activities, and participating in crisis management as needed. The Social Worker is an integral member of the multi-disciplinary treatment team. The Social Worker participates in interdisciplinary treatment planning; completes psychosocial evaluations of admitted patients and meets the documentation and turn-around-time standards. The Social Worker facilitates the discharge planning process on day one of treatment and maintains communication with collateral resources.
