Social Worker - LMSW, Kidney Transplant

The Complex Care Kidney Transplant Social Worker plays a vital role in supporting patients and families through the kidney and pancreas transplant journey. In this role, you’ll provide specialized psychosocial assessment, care coordination, and education within a fast‑paced, highly collaborative transplant environment.

You’ll work closely with transplant coordinators, financial teams, and a multidisciplinary care team to help patients prepare for transplant, navigate insurance and logistical challenges, and remain engaged throughout evaluation, waitlist, and post‑transplant phases.
