Summary The Fargo VA Health Care System is recruiting for 1 full-time Social Worker (Program Coordinator) to serve as Advanced Care Planning Coordinator and PACT Social Worker. This position is responsible for the coordination and facilitation of Advanced Care Planning Group Visits (ACP-GV), including contacting attendees by telephone within the required time frame for follow-up and tracking and monitoring ACP-GV's goal areas.
