Enhance client care as a Virtual Care Coordinator Registered Nurse with Home Instead, focusing on weekend assessments and care setup. Additional weekday evening shifts may be available.
Home Instead is looking for an experienced RN to conduct virtual assessments across various locations, primarily on weekends. This essential role supports seamless client intake and care coordination, working collaboratively with service coordinators. Strong communication and organization skills are critical for maintaining accurate documentation and facilitating effective problem resolution.
Key Responsibilities:
• Conduct virtual assessments and consultations with clients
• Collaborate with on-call teams to set up care services
• Develop and implement initial care plans for client satisfaction
• Oversee virtual introductions of clients with caregivers
• Document all client communications and assessments accurately
Requirements:
• Bachelor’s Degree in Nursing or equivalent
