This role is an exciting opportunity for a highly motivated and dedicated community clinician with experience of supporting learning, encouraging clinical curiosity, and confidently capable of assessing and responding to patients with complex long‑term conditions.

Working as part of the Ageing Well development team, you will be encouraged to help design and deliver evidence‑based knowledge about frailty and ageing well, understanding the importance of functional fitness, managing disease and coaching conversations.
