We are seeking an experienced Licensed Master Social Worker (LMSW) to provide psychosocial assessments, care coordination, patient advocacy, and resource management services in a Home Health setting. The ideal candidate will have prior skilled home health experience and be comfortable working independently in the community.

• Day Shift


• 8-Hour Shifts


• Monday–Friday Schedule


• No weekends required


• No on-call requirement

• Minimum 1 year of Skilled Home Health experience required


• Oasis experience required


• First-time travelers accepted
