Summary The VA Northeast Ohio Healthcare System is recruiting for a Licensed Practical Nurse. The Licensed Practical Nurse will function in Home Based Primary Care (HBPC) section within Community Outpatient Services (COPS). Responsibilities VA Careers - Licensed Practical Nurse: Total Rewards of a Allied Health Professional Conducts independent home visits for Veterans with complex medical and psychosocial needs, delivering direct nursing care and providing education to patients and caregivers in alignment with the individualized care plan established by the RN Case Manager. Collaborates with the HBPC Interdisciplinary Team including the Physician Preceptor, Registered Nurse, Licensed Practical Nurse, Occupational Therapist/Physical Therapist, Social Work, Dietitian, Speech Therapist, Psychologist, Pharmacist and Advanced Practice Provider.
