Description
PRIMARY PURPOSE:
To provide comprehensive, patient-centered nursing care within FoundCare's Federally Qualified Health Centers (FQHC), supporting primary care delivery, chronic disease management, preventive services, and care coordination for diverse and underserved populations, including patients with complex medical and social needs, under the supervision of a Registered Nurse (RN) or provider.
ESSENTIAL JOB FUNCTIONS:
Direct Patient Care

• Perform patient intake, including vital signs, medical history updates, and medication reconciliation.
• Administer medications, immunizations, and injections per provider orders and standing protocols.
• Conduct point-of-care testing, including glucose, rapid tests, and urine dipstick.
