30% Hematology
70% Oncology
Number of Physicians in Practice: 1 remaining physician
Hem/Onc only practice
Adults only
No Radiation Oncology on-site, but connection to Rad Onc team at Loyola University Medical Campus, which is 5 miles from practice.
No APP's in practice
Documentation System/EMR: epic
Estimated Credentialing Timeframe: 3 months
Practice Setting: Outpatient Free-Standing Community Cancer Center
Average Number of Patients Per Day: 16-20
Submission Requirements

• Board Certified - Required
• Active IL license - Required at time of name clear
• Availability - Please provide at time of name clear - must be available FT
