Benchmark's Supported Living program assists adults with daily activity skills to gain independence in small personal home settings.
Benchmark is seeking an LPN Healthcare Coordinator to provide healthcare coordination and direct intervention, as needed, for designated individuals in our Supportive Living Program. The nurse will also provide training for unlicensed staff to meet the healthcare needs of individuals served. Our diverse and dynamic programs can't operate efficiently without the strength and heart of our nurses!
