The LPN Home Health provides nursing care to patients of all ages in the home under the direction and supervision of the RN case manager in accordance with the plan of care and physician orders. • Provides skilled nursing care and treatments to patients of all ages as assigned by the RN.
• Assists the RN case manager in coordinating all aspects of patient care.
• Completes and submits accurate clinical documentation within agency timelines.
• Complies with all hospital, departmental, and regulatory policies and procedures.
• Participates in continuing education, staff meetings, and required competencies.
• Provides customer service with respect, kindness, and empathy to patients, families, and staff.
