**Job Summary and Responsibilities**

We are looking for a compassionate and skilled Licensed Practical Nurse (LPN)/Licensed Vocational Nurse (LVN) to join our team and help embody this mission. As an LPN you will deliver direct patient care based on approved treatment plans while adhering to clinical protocols and guidelines.

+ Collaborate and coordinate with the RN Case Manager and other healthcare team members to ensure effective patient care according to the treatment plan and patient care guidelines.
+ Conduct patient visits as specified by the physician’s treatment plan, ensuring compliance with federal, state, and county regulations.
