In this role, under the supervision of a clinical manager, you will assess, plan, and implement patient care, educate patients and their families on disease processes, procedures, and medications, and manage various patient needs such as wound care, trachs, Gtubes, Catheters, Vents, and medication administration. We create customized care plans to accommodate our patient's needs, preferences, and concerns, as well as special requests of their families.
