**Job Summary and Responsibilities**
As our Licensed Practical Nurse (LPN), you will help our patients by providing nursing care to our skilled nursing residents under the direction of the physician or registered nurse. Every day you will work with our patients to evaluate their needs and provide physical or psychosocial care. To be successful in this role, you must have a genuine compassion for your patients’ well-being and understand the role you play in helping our our patients feel at ease. You also have the patience to explain the process, answer questions and update documentation.
+ Patient Admissions & Care Planning: Collaborates with the Charge Nurse on admission orders and medication sheets, assists in developing and updating care plans based on patient condition changes
+ Patient Assessment & Reporting: Performs assessments when notified of patient condition changes, reporting findings to the Charge Nurse and providers as requested
