Continuum Hospice is seeking a dynamic and dedicated nurse to join our team as a LVN Hospice Liaison. If you have a background as a hospice clinician, a strong interest in expanding your career into marketing and sales, and are passionate about our hospice mission, we invite you to consider this exciting opportunity.

As a LVN Hospice Liaison, you will support and partner with current Liaisons to identify new referral sources and maintain ongoing relationships to increase program census and preference for hospice services. LVN Hospice Liaison use their clinical knowledge, empathetic communication skills, and marketing abilities to initiate one-on-one dialog with referral sources, families, and prospective patients to help explain and educate on hospice services. Our LVN Hospice Liaison's are hospice experts and serve as a resource to our Sales team and referral sources to ensure extraordinary patient and customer service.
