MDS Coordinator Restorative Director

The RN or LPN must be proficient and have MDS experience

This is a dual role and the candidate will be both the MDS Coordinator as well as the Restorative Coordinator. The candidate must be proficient in MDS and experienced in Restorative Therapies. This opportunity is open to a Registered Nurse (RN)or Licensed Practical Nurse (LPN), licensed in the State of Illinois, who assumes the primary responsibilities for the care planning process and MDS completion within this facility and acts as the facilitator at the conference meeting.
